This form is used to make a request to pay a coach for their time spent with a student group. You must previously have spoken to the Students Union Personnel Department to fill out all of the necessary employment forms and have gained a payroll staff number for your coach. Their hours worked must then be completed and signed off by the group Treasurer and the coach themselves.
Must NOT be deposited in the drop-box.
